Position Essential Duties/Responsibilities:

• Works closely with the Director of Personal Care, day shift charge nurses and alternate shift RN Campus Supervisors for effective 24-hour management.
• Recognizes and responds to significant changes in resident condition and provides leadership in any emergency.
• Rounds daily on each house, ensuring level of care needs are met.
• Provides clinical support to the team members.
• Oversees quality issues, monitors incidents/accidents, provides follow-up monitoring, and addresses issues immediately.
• Works with RH Infection Control Preventionist in monitoring infection control issues in assigned house(s), completes forms and reviews reports.
• Assists with direct care, such as bathing, assists in the dining room, responds to call bells and participates in resident activities as able.
• Serves as a change agent, communicating change positively to staff.
• Is available (on-call) in an emergency situation when required staffing is affected.
• Shares weekly on-call with the Director of Personal Care.
• Works 2 shifts per month on the floor as a nurse.
• Works with scheduler on staffing issues to ensure that call-offs and vacancies are filled to provide adequate coverage as per regulatory requirements and resident requirements.
• Coordinates monthly billing assessments with Personal Care households,and submits billing to the Finance department.

Qualifications

LPN Qualifications

• Nursing/healthcare experience required
• Licensed in the state of PA
• Current CPR certification
