Job Title:Patient Navigator / Medical Assistant

Location: Sheridan Community Hospital, Sheridan, MI

Type: Full-Time Monday-Friday

About Us:

Sheridan Community Hospital is seeking a compassionate, organized, and motivated Patient Navigator/Medical Assistant to join our team. This position works closely with patients, families, providers, and community resources to coordinate care and help patients successfully navigate their healthcare needs.

Position Summary:

The Patient Navigator/Medical Assistant supports a team-based approach to care focused on improving health outcomes and providing coordinated, high-quality, patient- and family-centered care.

Responsibilities:

  • Contact patients following hospital and emergency department discharges to assist with follow-up care and appointments.
  • Conduct patient assessments, including PHQ-9 and Social Determinants of Health (SDOH) screenings.
  • Serve as a patient advocate and help identify healthcare and social needs.
  • Coordinate care among patients, families, physicians, advanced practice providers, specialists, payers, and community resources.
  • Assist patients and families with managing chronic health conditions and improving health literacy.
  • Develop and monitor individualized care plans in collaboration with patients, families, and providers.
  • Connect patients with appropriate medical, specialty, and community resources.
  • Support ACO/PCMH care coordination and population health initiatives.
  • Participate in quality improvement initiatives and monitor established quality and patient safety measures.
  • Maintain accurate documentation within the electronic health record.
  • Maintain patient privacy and confidentiality in accordance with HIPAA requirements.
  • Participate in required Care Management training, meetings, and professional development activities.

The position serves as a key contact and resource for patients and their families throughout the care coordination process.

Qualifications

Qualifications:

  • High school diploma and graduation from a Medical Assisting program required.
  • Associate degree preferred.
  •  2+ years of clinical experience as a Medical Assistant, CNA, or similar clinical role preferred.
  • 2+ years of clinical informatics or similar experience preferred.
  • Knowledge of medical practice operations and basic patient care.
  •  Excellent written and verbal communication skills.
  •  Strong organizational skills with the ability to manage multiple priorities and deadlines.
  • Proficiency with Microsoft Office, including Word, Excel, and Outlook.
  • Ability to learn and work within multiple electronic medical record systems.
  • Strong attention to detail, problem-solving skills, and the ability to work effectively as part of a healthcare team.
