Position Summary

We are seeking a dedicatedPhysical Therapist (PT) to join our in-house rehabilitation team. In this role, you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services designed to improve mobility, strength, function, and independence.

Key Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services in accordance with physician orders
• Monitor resident progress and adjust treatment plans as needed
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members
• Complete accurate and timely documentation
• Participate in care plan meetings and discharge planning
• Educate residents and caregivers on exercises, safety, and mobility strategies
• Maintain compliance with all regulatory and professional standards
• Support positive resident outcomes and quality-of-life goals

Qualifications

• Available Sunday through Thursday (weekend availability required)
• Degree from an accredited Physical Therapy program
• CurrentSouth Carolina Physical Therapist license in good standing
• Long-term care or skilled nursing experience preferred
• Experience withPointClickCare (PCC) andCasamba preferred
• Strong clinical, communication, and organizational skills
• Must maintain all required continuing education and licensure requirements
