St. Ann’s Community is currently looking for a dynamic part time life enrichment care partner for the St. Ann's Care Center. The purpose of this job is to enhance their quality of life through coaching and collaboration with front line staff. You will work closely with the interdisciplinary team to provide compassionate and relationship based person-centered care.

Job Description:

• Follows individualized plan of care, promoting maximum level of wellbeing
• Uses interventions appropriate to interest, needs and functional level of resident
• Observes for behavioral changes and alters approaches to meet resident needs
• Evaluates interventions to assure efficiency and effectiveness in meeting residents’ needs
• Provide clear and concise directions and adaptations as needed to maximize independence in recreation and leisure activities
• Provide a variety of meaningful programs, as deemed by Life Enrichment Coordinator and resident(s), that emphasize individuals’ strengths and abilities, sustain or improve physical and psycho-social functioning, offer sensory and intellectual stimulation and promote a sense of usefulness and community spirit
• Maintain records, such as attendance and inventory of supplies
• Assist in coordinating SACC special events with resident(s) and staff
• Support community wide events and/or intergenerational programming with resident(s) and other staff members
• Monitor and maintain supplies necessary for program implementation within the recreation budget
• Coordinate and supervise community outings with residents and other staff members
• Evaluates programs to assure efficiency and effectiveness in meeting residents’ needs
• Honors Residents food choices
• Responsible for preparation/serving of Residents meals and snacks (as per meal tickets) and proper kitchen clean up
• Will provide meal substitutes according to Resident’s diet
•  Will maintain and log proper food and refrigerators temperatures
•  Consistently follow standard principles and universal precautions to preserve food quality and prevent the transmission of foodborne illness.
• Maintains timely participation documentation of events and programs in Care Merge
• Informs residents of resident group meetings including: Resident Council and on-sight groups
• Supervise volunteers and support volunteers by providing clear direction and adequate support
• Attend and participate in staff meetings and educational opportunities
• Provides rotating weekend life enrichment coverage
• Provides rotating evening life enrichment coverage
• Required to become CPR certified within 3 months of hire and required to maintain certification annually thereafter.

Candidate Requirements:

• Two-five years’ experience working in a senior living community.

• Experience leading recreation/leisure activitiespreferred.

• Active Certified Nursing Assistant licensepreferred.

   

• High School Diploma/GEDrequired.
