Remedy Home Health Care is actively seeking alicensed Physical Therapist (PT) to join our growingHome Health team in Orange County.

This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to providehigh-quality rehab and community-based care directly to patients in their homes.

As aHome Health Physical Therapist, you will work withsenior care populations, including geriatric patients recovering from illness, injury, or surgery. You’ll deliverskilled therapy services inhome-based, assisted living, and community care settings, helping patients regain independence and improve their quality of life.

Your Role: Home Health Physical Therapist (PT)

As aPhysical Therapist in Home Health, you will provideskilled rehab services tailored to each patient’s needs. Your focus will includefall prevention, strength building, mobility training, and functional independence forsenior and geriatric patients.

Key Responsibilities:

• Conductin-home physical therapy evaluations and reassessments
• Develop and implementpersonalized rehabilitation care plans
• Improvestrength, balance, mobility, and functional outcomes
• Provide therapy inhome health, assisted living, and community care settings
• Educate patients and families onfall prevention and safe mobility techniques
• Document visits usingWellSky (Kinnser) EMR
• Collaborate withinterdisciplinary home health teams

Qualifications

• Licensed Physical Therapist (PT) – California (Required)
• CPR Certification (AHA approved)
• Minimum1 year of clinical experience (Home Health preferred)
• Experience insenior care, geriatrics, rehab, or community-based care preferred
• Strongclinical, communication, and patient education skills
• Validdriver’s license, reliable transportation, and auto insurance
