About the Position 

The CNA supports our nursing and medical team with patient care, medicalrecords and appointment coordination. This position also supports patients served through the Department of Corrections (DOC) and county jails by preparing and tracking medication chain-of-custody paperwork. We are looking for someone who is dependable, organized,accurate and respectful of the people we serve. 

Responsibilities 

• Welcome androom patients; obtain and document vital signs and promptly report concerns to the nurse. 

• Prepareaccurate DOC and county jail chain-of-custody forms from nurse-approved records and route them to nursing for review. 

• Track outgoing and returned forms, check required signatures and dates, follow up on missing information, and upload paperwork to the correct patient chart. 

• Maintain the annual physical appointment list. Work with the front desk,nurse and nurse practitioner to schedule exams before they are due, reschedule missedvisits and confirm completed documentation. 

• Scan and upload medical records,maintain trackinglogs and notify nursing of documents or concerns needing review. 

• Prepare and clean exam rooms, restock supplies, andassist with assigned specimen-handling tasks after training. 

• Protect confidentiality and provide calm, nonjudgmental support using trauma-informed approaches. 

This is a supervised support role. Medication preparation, dispensing, routineadministration and independent clinical decisions are not part of this position. 

Required Qualifications 

Active New Jersey CNA certification in good standing; current CPR/BLS; reliable attendance and punctuality;accurate data entry and basic computer skills; and comfort working with people receiving mental health and substance use treatment. 

Preferred Qualifications 

Experience in outpatient care,an opioid treatmentprogram, communityhealth orcorrectional-program support. Familiarity with electronic health records, infection control, specimenhandling or chain-of-custody documentation. Bilingual English/Spanish is a plus.
