Position Overview
We are seeking aCare Coach 1 to support members in achieving and maintaining optimal wellness by assessing their needs, identifying barriers to care, and connecting them and their families with appropriate healthcare and community resources.
The ideal candidate will have prior experience working as aCare Coach for *** within the past few years. Previous experience in this role may help reduce the training timeline and support a quicker onboarding process.
This position is consideredpatient-facing and involves local travel throughout Pasco County.
Key Responsibilities
- Assess and evaluate members' needs and requirements to support optimal health and wellness outcomes.
- Guide members and families toward appropriate healthcare, social service, and community resources.
- Employ a variety of strategies, approaches, and techniques to address members' health and wellness needs.
- Identify and resolve barriers that may hinder effective care.
- Monitor members' progress toward desired outcomes through assessments, data review, conversations with members, and active care planning.
- Facilitate interaction between members and appropriate healthcare or community resources.
- Maintain accurate and timely documentation using electronic case management and other computer systems.
- Follow established professional standards, regulations, procedures, and organizational guidelines.
- Make decisions regarding work priorities and approach while following established direction and procedures.
- Support an organization focused on continuously improving the consumer experience.
Required Qualifications
- Bachelor's degree.
- 2+ years of related experience.
- Proficiency with Microsoft Office, including Word and Excel.
- Exceptional verbal, written, and interpersonal communication skills.
- Ability to use a variety of electronic information applications and software programs.
- Passion for contributing to an organization focused on continuously improving consumer experiences.
- Valid state driver's license.
- Proof of personal vehicle liability insurance with minimum coverage limits of$100,000/$300,000/$100,000.
- Ability and willingness to travel throughout Pasco County.
- This position is patient-facing and requires participation in the organization'sTuberculosis (TB) screening program. Selected candidates will be required to complete TB screening.
Preferred Qualifications
- Master's degree.
- Prior experience working withMedicare and Medicaid recipients.
- Previous experience with electronic case-note documentation.
- Experience documenting information across multiple computer applications and systems.
- Experience in health promotion, coaching, and wellness.
- Knowledge of community health and social service agencies.
- Familiarity with local and additional community resources.
- Previous experience as a Care Coach for *** within the past few years.
