Overview:
The Housing Case Management Specialist is responsible for supporting those living with HIV and the Matthew 25 team to ensure that patients are assessed for housing needs are connected to housing resources as applicable. The Housing Case Management Specialist will accomplish this by utilizing Matthew 25ās Kentucky Ryan White Part B housing funds, and funds derived from Kentucky HUD housing programs, HOPWA and HOME TBRA. These programs provide support to clients who are at-risk for homelessness and those who are homeless.
QUALIFICATIONS:
- Bachelorās degree in Social Work
- Interpersonal skills and ability to engage and work effectively with diverse staff and client population is required.
- Leadership experience in the workplace setting, minimum of 2 years preferred.Ā
- Experience working in a nonprofit organization or a strong desire to work in service driven environment.Ā

- Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
- Must be able to pass a drug screen and background test
