ABOUT THE ROLE

WeareseekinganexperiencedandcompassionateRegisteredNursetojoinourSalvosHomeCareteamuntil31January2028.Inthisrewardingcommunity-basedrole,youwillprovideclinicalleadership,carecoordinationandnursingsupporttoclientslivingindependentlyintheirownhomes.

WorkingcloselywithCarePartners,HomeCareWorkers,familiesandexternalhealthcareproviders,youwillplayakeyroleinensuringclientsreceivesafe,person-centredcarethatpromotestheirhealth,wellbeingandindependence.

KEY RESPONSIBILITIES

AsaRegisteredNurse,youwill:

• Conductcomprehensivenursingassessmentsanddevelop,implementandreviewindividualisedcareplansforclientslivingathome.
• Monitorchangingclinicalneedsandcoordinatetimelyinterventionstosupportpositivehealthoutcomes.
• LiaisewithGPs,alliedhealthprofessionals,clientsandfamiliestoensureintegratedandresponsivecare.
• Maintainaccurateclinicaldocumentation,careplans,incidentreportingandriskassessmentsinlinewithorganisationalandregulatoryrequirements.
• ProvideclinicalguidanceandsupporttoHomeCareWorkers,promotingsafeandeffectivecarepractices.
• Participateincaseconferences,qualityimprovementinitiativesandcontinuousprofessionaldevelopmentactivities.

ABOUT YOU

Tobesuccessful,youwillhave:

• CurrentunrestrictedAHPRAregistrationasaRegisteredNurse.
• Minimumtwoyears'post-graduatenursingexperience,ideallywithinagedcare,communitynursingorhomecareservices.
• Strongclinicalassessmentandcareplanningskills.
• Excellentcommunicationandrelationship-buildingabilities.
• SoundunderstandingofAgedCareQualityStandardsandperson-centredcareprinciples.
• Acurrentdriver'slicenceandwillingnesstotravelthroughoutthelocalcommunity.
