Who We Are and Why Join Us
At OnMed our purpose is simple but powerful...to improve the quality of life and sense of well-being in our communities by bringing access to healthcare to everyone, everywhere. Our path to everywhere has already begun, with our innovative CareStation, a small but mighty, Clinic-in-a-Box, bringing #healthcareaccess anywhere with an outlet to plug it in. Poised to become a key component in America’s public health infrastructure, the OnMed CareStation is the only tech-enabled, AI-powered, human-delivered, hybrid care solution that combines the comprehensive experience, trust and outcomes of a clinic,  with the rapid scalability of virtual care. Â
At OnMed, every role, everyday, is directly impacting the communities we serve. You’ll join a high-performing purpose-driven team, innovating to break down the barriers that keep people from the care they need. Â
This is not just a job...it's a movement to bring access to healthcare where and when people need it most. It’s healthcare that shows up.Â
Â
Who You AreÂ
You've spent your career at the intersection of purpose and practice — in community health outreach, nonprofit or public-health consulting, or social-impact strategy — helping organizations turn intention into measurable change. Â
This is not an account management role. You are the primary relationship holder for a portfolio of 20–25 CareStation partners. From onboarding forward, you and the partner set shared goals for the impact the CareStation should create — appropriate to each partner's objectives and circumstances — and you hold both OnMed and the partner accountable for reaching them. You bring what you learn in the field back to shape how OnMed builds and deploys.Â
Â
The RoleÂ
As Partner Impact Manager, you own the day-to-day relationship for a portfolio of CareStation partners — setting shared goals at onboarding, tracking outcomes, and bringing field insight back into how OnMed builds and deploys.Â
Reporting Structure
This role reports to our Partner Impact Director.Â
Key ResponsibilitiesÂ
- Serve as the primary relationship holder for a portfolio of 20–25 CareStation partnersÂ
- Guide partners through onboarding, embedding the CareStation into their broader community health strategy — not just installing itÂ
- At the start of each partnership, establish a clear, shared objective and mutually agreed goals appropriate to the partner's objectives and specifics — so success is defined up frontÂ
- Ensure clear mutual understanding of roles and responsibilities during the duration of the program and hold OnMed and the partner accountableÂ
- Track and analyze partner-level outcomes data to spot what's working, what's stalling, and where to focus nextÂ
- Run forward-looking, mission-grounded partner reviews — not backward-looking report-outsÂ
- Bring partner insight back into OnMed, shaping how the roadmap evolves to meet partner and community needsÂ
- Identify moments where a partner is ready to expand their commitment and help them build the case internallyÂ
- Contribute to OnMed's broader impact narrative by documenting community outcomes across your portfolioÂ
- Must be a native-level Spanish speaker, both verbally and in writing to effectively communicate with our Puerto Rico-based partnersÂ
Â
- Perform other related role's responsibilities as assignedÂ
Knowledge, Skills and AbilitiesÂ
- Demonstrated ability to influence without authority — you move partners toward shared goals through trust and clarity, not hierarchyÂ
- Experience helping organizations translate mission into measurable outcomesÂ
- Comfort operating in complex, multi-stakeholder environments — health systems, government agencies, community organizationsÂ
- Strong written and verbal communication — able to distill complexity for a C-suite audience and a community health worker in the same dayÂ
- Genuine personal commitment to health equity and community accessÂ
- Direct experience in community health, FQHCs, public health programs, or social determinants of health initiatives a plusÂ
- The confidence to hold a partnership — yourself and your partner alike — to a high bar, while deepening the relationshipÂ
- Comfortable working in data and analytics tools to track partner health, surface trends, and back up recommendations with numbers, not just instinctÂ
Education and ExperienceÂ
- Bachelor's degree required; degree in Public Health, Public Policy, Social Work, or a related field preferredÂ
- 5+ years in a client-facing or partner-facing role within community health outreach, nonprofit or public-health consulting, or social-impact strategyÂ
- Background in management consulting, nonprofit strategy, or impact investing with a client-facing componentÂ
- Experience working with underserved communities in a professional capacityÂ
- You must be fluent in Spanish with professional-level verbal and writing skillsÂ
- Role requires with occasional travel up to 30-40% of timeÂ
OnMed provides a competitive salary and benefits package, including unlimited PTO and paid holidays.Â
The base salary for this role is up to $120,000 commensurate with the candidate's experience.
OnMed is a proud equal opportunity employer. All qualified applicants will be considered without regard to race, color, creed, religion, gender, sexual orientation, national origin, genetic information, disability, age, marital status, veteran status, or any other category protected by law.Â
