About the role
The Aging at Home (AAH) program, offered through the Healing and Wellness Branch, supports Métis Nation of Ontario (MNO) Citizens aged 55 and older to maintain their independence, wellness and quality of life while remaining safely in their own homes. The program provides a range of in-home and community-based supports, including light housekeeping, meal preparation, laundry assistance, friendly visiting and supportive transportation for essential errands such as grocery shopping, banking and pharmacy visits.
The AAH Coordinator supports clients in accessing services and direct supports, including gas cards or third-party transportation for specialized medical appointments based on pre-determined program criteria. The Coordinator also coordinates and tracks contracted services such as lawn care and snow removal. Through relationships with local service providers, community partners and MNO teams, the AAH Coordinator contributes to accessible, culturally relevant services that promote the wellness, independence and quality of life of MNO seniors.
What you will be doing
- Ensure that the Aging at Home program is delivered in the spirit and philosophy of the Métis Nation of Ontario’s Statement of Prime Purpose.
- Lead clients through a supportive process to identify immediate needs and develop clear wellness plans.
- Review supporting documentation and assess client eligibility in accordance with program criteria.
- Provide client services, including in-home supports, friendly visits and service coordination with outside agencies, where applicable.
- Coordinate, track and process contracted third-party services for in-home supports, lawn care and/or snow removal, where applicable.
- Coordinate, track and process direct client supports in accordance with program requirements.
- Support clients in accessing gas cards or third-party transportation for specialized medical appointments.
- Calculate financial supports based on pre-determined program variables and program parameters.
- Assist clients in accessing appropriate services through referrals, where applicable.
- Maintain accurate client service documentation in the Online Health and Access Tracking System (OHATS), Data Basics and Excel tracking documents, as required.
- Complete financial reporting related to local program site budget allocations and established parameters.
- Develop and maintain positive relationships with MNO Citizens, employees, Council, volunteers, advocates, community partners and service providers.
- Reflect Métis cultural approaches in the delivery of activities and services.
- Participate in MNO Healing and Wellness training and other required training as determined.
- Participate in ongoing professional development activities as time and financial resources permit.
- Attend training and meetings outside Métis Nation of Ontario working hours, where required.
- Maintain confidentiality at all times and adhere to applicable privacy legislation.
- Report unsafe conditions, hazards, practices, accidents and incidents in accordance with the Occupational Health and Safety Act, MNO policies and procedures, and applicable provincial legislation.
- Perform administrative duties and other job- or branch-related duties as required.
Qualifications:
- College diploma (1 or 2 year), preferably in social services, community services or a related field.
- 1 to 3 years of relevant work experience.
- 1 or more years’ experience providing client services.
- 1 or more years’ experience working with seniors aged 55 and older.
- Experience working with volunteers in a non-profit environment.
- Knowledge of the Métis Nation of Ontario and programs and services offered by MNO.
- Knowledge of Indigenous programs and services.
- Proficiency with computer programs, including Outlook, Excel, Word and PowerPoint.
- Ability to work independently with minimal supervision and as part of a team.
- Well-developed administrative, organizational and communication skills, with the ability to meet deadlines.
- Previous experience working with confidential information.
- Ability to lift up to 11.5 kg when required.
- Reliable vehicle, or ability to rent a vehicle, valid driver’s licence, $2,000,000 liability insurance and a clean driving record.
- Ability to complete a Police Records Check and Vulnerable Sector Check.
- Current CPR-AED certification.
Preferred:
- Knowledge of Métis-specific and broader Indigenous policy, research, key challenges and programs.
- Previous experience working with Métis communities or demonstrated knowledge and experience of Métis culture.
- Previous experience working within a trauma-informed care setting.
- Demonstrated ability to quickly acquire and expand knowledge and remain flexible in a demanding and dynamic environment.
