Memorial Community Hospital & Health System (MCH) is seeking a compassionate, experienced, and clinically skilled RN or LPN to join our Rural Health Clinic team. This vital role serves as the primary point of contact for patients requiring telephone, electronic, and in-person triage services. The ideal candidate will excel in clinical assessment, critical thinking, patient education, and care coordination while helping ensure patients receive the right care at the right time.
The Clinic Triage Nurse serves as an interdisciplinary team member responsible for assessing and coordinating patient care through telephone, electronic, and in-person triage. Using established protocols, nursing assessment skills, and sound clinical judgment, the nurse evaluates patient needs, determines appropriate care recommendations, facilitates timely access to services, and supports continuity of care throughout the Rural Health Clinics.
What You’ll Do:
- Shift Details: Monday-Friday 8:00a-4:30p, 40 hours per week.
- Conduct comprehensive telephone, electronic, and in-person triage assessments.
- Recognize routine, urgent, and emergent patient needs and determine the appropriate level of care.
- Provide home care guidance, schedule appointments, coordinate same-day visits, consult with providers, and facilitate emergency care when necessary.
- Manage clinic communication workflows, including:
- Telephone encounters
- Patient portal messages
- Medication refill requests
- Provider and nursing message pools
- Test result communication
- Care coordination activities
- Review and process medication refill requests according to approved protocols and provider direction.
- Assess walk-in patients presenting with acute concerns and obtain vital signs and pertinent clinical information.
- Identify signs and symptoms requiring immediate intervention and activate emergency response procedures when indicated.
- Communicate test results and provider recommendations while ensuring patient understanding.
- Assist patients in navigating MCH services, specialty referrals, population health resources, and community services.
- Maintain accurate, timely, and complete documentation within the electronic health record (EHR).
- Collaborate with providers, medical assistants, nursing staff, population health teams, and other departments to support efficient patient care.
What We're Looking For:
Education
- Graduate of an accredited Nursing Program.
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Nebraska, Iowa, or compact state of residence.
Certifications
- Basic Life Support (BLS) certification required.
- Advanced Cardiovascular Life Support (ACLS) certification required or must be obtained within 90 days of hire.
Experience
- Minimum of 3 years of nursing experience required.
- Previous experience in hospital, ambulatory care, primary care, family practice, or health clinic settings preferred.
- Telephone and in-person triage experience preferred.
- Experience with Epic or other Electronic Health Record (EHR) systems preferred.
