JOB SUMMARYÂ
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessibleÂ
and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemicÂ
barriers to healthcare and promote health equity for all. We offer a wide range of services, includingÂ
mental health care, residential and outpatient substance use treatment, and primary health services.Â
Additionally, we provide transitional support for individuals re-entering the community after involvementÂ
in the criminal justice system. By integrating physical and behavioral health, we empower individuals toÂ
overcome challenges by addressing social determinants of health, fostering resilience, and facilitatingÂ
recovery.
The 214 Men’s Residential Step Down (MRSD) is a program of HealthRIGHT 360 located at Alamo SquareÂ
neighborhood in San Francisco. It is a 64-bed male transitional housing for individuals that have completedÂ
at a minimum of 30 days residential substance use disorder treatment. MRSD provides temporary housingÂ
to assist our clients in their next stage of sobriety.Â
The On-Cal Recovery Coach is responsible for assessing client needs and strengths in support of healthÂ
and recovery. Through individual coaching with program specific requirements and case management,Â
On-Call Recovery Coaches assists clients in navigating systems of care while providing a supportiveÂ
recovery environment. On-Call Recovery Coaches are responsible for maintaining and assessing the safetyÂ
and security of the program and its residents through continued house runs while providing a supportiveÂ
environment. Through effective communication, On-Call Recovery Coaches relay relevant information toÂ
other members of the interdisciplinary team and perform crisis intervention as needed. On-Call RecoveryÂ
Coaches ensure that the facility environment is safe and welcoming.
KEY RESPONSIBILITIESÂ
Individual Responsibilities:Â
• Provides direct services such as intakes, assessment, crisis intervention, and case management asÂ
needed.Â
• Responsible for retention and completion of clients, facilitates individual coaching sessions withÂ
each caseload client.Â
• Provides learning experience opportunities and offers support to assist clients in meeting theirÂ
long-term recovery goals.Â
• Proactively links clients to both internal and external resources based on their recovery needs andÂ
follows up on the progress/status.Â
• Performs house runs to ensure and maintain accountability of all clients and the safety andÂ
security of the facility.Â
• Performs crisis intervention and communicates with treatment team as unforeseen situationsÂ
arise.Â
• The Recovery Coaches documents client updates and incidents in the facility log daily, performsÂ
CPR and First Aid as needed, and works weekends and holidays as needed.
Setting Responsibilities:Â
• Facilitates community meetings and ensures compliance with facility rules.Â
• Performs crisis intervention and communicates with team as unforeseen situations arise.Â
• The Recovery Coach will document all client updates and facility incidents daily in the facility log.Â
• Performs periodic house runs to ensure and maintain the safety and security of the facility.Â
• When assigned the DOT, documents, and accurately distributes client money, ensures clientÂ
medications are securely stored and properly accounted for and holds facility keys and floaterÂ
cellular phones.Â
• As needed, accompanies clients to offsite appointments related to their recovery.Â
• Attends and completes all required trainings and meetings as assigned by facility manager.Â
• Plans and facilitates client celebrations and special events.Â
• Recovery Coaches must be available to work weekends and holidays as needed.Â
Documentation Responsibilities:Â
• Collaborates with each client and other available internal and external resources to developÂ
transition plans, progress notes and appropriate updates in support of the health and recoveryÂ
needs of the client.Â
• Properly documents all individual encounters and completes the discharge paperwork/processÂ
and required agency assessments in timely manner.Â
• Also, maintains accurate records by data entering documentation into various electronic systemsÂ
for all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal andÂ
external evaluating requirements.Â
And perform other duties as assigned.Â
QUALIFICATIONS
Education, Credentials, and Experience
• High School Diploma or equivalent, bachelor’s degree preferred.
• Drug and Alcohol registration or certification recognized by Department of Health Care ServicesÂ
(DHCS) preferred.
• First Aid and CPR Certified within 30 days of employment.
• Valid California Driver’s License and automobile insurance.
• Experience working successfully with diverse populations specifically with issues of substanceÂ
abuse, mental health, criminal background, and other potential barriers to economic self sufficiency.
Knowledge and Skills
• Knowledge of gender-responsive, trauma informed, and co-occurring treatment.
• Strong computer skills, especially with Microsoft Office applications and internet applications.
• Ability to work within a frequently changing project scope while maintaining overall direction andÂ
structured priorities.
• Self-motivated, dependable, creative, and proactive approach to work; understands importanceÂ
of working independently and within a team environment.
• Ability to enter data into various electronic systems while maintaining integrity and accuracy ofÂ
data.
• Excellent verbal and written communication skills and interpersonal skills.
• Ability to handle sensitive information in a confidential manner.
• Bilingual in the SF county language threshold desired.
