We have a great opportunity for an Aboriginal or Torres Strait Islander Identified Aged Care Assessor to join our Aged Care Assessment Services (âACASâ) team on the Mid North Coast. ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region. Â
What will your role look like?Â
You will be conducting assessments to identify and meet the needs ofAboriginal and Torres Strait Islander Identifiedclients and refer to appropriate home support services. Â
Day to day this includes:Â Â
- Collaborating with clients, families and multidisciplinary teams as requiredâŻÂ
- Working collaboratively with colleagues to promote a supportive and positive team cultureâŻÂ
- Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support servicesâŻâŻÂ
- Reviewing support plans to meet clientâs changing support needsâŻÂ
- Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframesâŻÂ
- Maintaining currency of knowledge relating to aged care services both funded and non-fundedÂ
- Assisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomesâŻâŻÂ
To be successful in this role youâll need:Â
- An Australian Driverâs license and access to a registered, insured vehicle Â
- Excellent communication skills, both written and verbalÂ
- Commitment to excellence in customer service, both to internal and external stakeholdersâŻÂ
- Excellent planning, organising and problem-solving skillsâŻÂ
- Previous knowledge or work experience in the Aged Care System preferredâŻÂ (but not essential)
- Demonstrated understanding of and to undertake person centred, goal directed, strength based holistic assessmentsâŻÂ (or transferable skills)
- Demonstrated understanding of work health safety principals including the required analytical skills to identify and effectively manage risksâŻÂ
- Willingness to apply for a Police CheckÂ
