Meritus Health is launching theMeritus Personal Health Institute (MPHI) — a first-of-its-kind population health and precision medicine initiative rooted in the rural communities of Western Maryland.

Backed by a$24M initial investment, MPHI will enroll approximately10,000 residents in a longitudinal program combining whole genome sequencing, advanced laboratory testing and imaging, pharmacogenomics, metabolic profiling, personalized health coaching, and comprehensive preventive health assessments —at no cost to the patient.

The aspirational goal is toincrease healthy life expectancy in rural Maryland by three years, while establishing Meritus Health as a national model for rural precision health.

Position Overview

The Nurse Practitioner will be an integral member of the MPHI clinical team, working closely with the Medical Director, APPs, health coaches, technicians, navigators, researchers, and physicians across Meritus Health.

This role will provide advanced practice clinical care throughout the participant journey, including comprehensive health assessments, interpretation and application of advanced diagnostic information, preventive health planning, risk-factor management, and longitudinal follow-up.

The NP will help translate the program's precision-health data intopractical, personalized recommendations that participants and their primary care teams can use to improve health and prevent disease.

This is a unique opportunity for an advanced practice clinician to help build a new clinical model from the ground up while contributing to a large-scale longitudinal population health initiative.

Key Responsibilities

• Provide comprehensive clinical assessments for MPHI participants as part of the program's standardized intake and longitudinal care model.

• Conduct medical histories, physical examinations, medication reviews, preventive health assessments, and risk assessments.

• Review and interpret laboratory, genomic, pharmacogenomic, imaging, cardiopulmonary, metabolic, and other diagnostic results within the NP's scope of practice.

• Translate complex health and precision-medicine findings into understandable, actionable recommendations for participants.

• Develop individualized preventive health and wellness plans in collaboration with participants and the MPHI interdisciplinary team.

• Identify health risks and clinical abnormalities requiring additional evaluation, treatment, or referral.

• Manage appropriate chronic disease and risk-factor interventions in collaboration with the Medical Director and participants' existing care teams.

• Utilize pharmacogenomic information and other precision-health data to support medication management and clinical decision-making.

• Work with the MPHI team to establish appropriate clinical protocols, workflows, standing orders, and standardized approaches to participant care.

• Participate in the development and refinement of the2.5-hour participant intake and assessment workflow.

• Coordinate referrals to primary care, specialists, behavioral health, nutrition, physical therapy, and other appropriate services when indicated.

• Communicate clinically relevant findings with participants' existing physicians and other members of the care team.

• Collaborate with health coaches and navigators to support participant adherence to personalized health recommendations.

• Participate in longitudinal monitoring of participant outcomes through laboratory testing, wearable technology, health assessments, and other available data.

• Assist with identifying changes in participant health status that require clinical intervention or escalation.

• Document clinical assessments, recommendations, care plans, and follow-up activities accurately within Epic.

• Support development of Epic workflows involving clinical decision support, pharmacogenomic alerts, referrals, and information sharing.

• Participate in quality-improvement initiatives designed to evaluate and improve MPHI clinical workflows and participant outcomes.

• Collaborate with Meritus School of Osteopathic Medicine faculty and research personnel on approved research, publications, presentations, and grant-supported initiatives.

• Participate in education and outreach activities involving Meritus physicians, clinical teams, and community partners.

• Maintain current knowledge of developments in precision medicine, genomics, preventive medicine, lifestyle medicine, population health, and advanced diagnostics.

• Practice in accordance with applicable Maryland regulations, organizational policies, professional standards, and MPHI clinical protocols.

Clinical Focus

The NP will work within a comprehensive precision-health assessment model that may include:

• Whole genome or exome sequencing and polygenic risk assessment

• Pharmacogenomic testing

• Coronary artery calcium scoring

• DEXA body composition and bone-density assessment

• VO₂ max and cardiopulmonary exercise testing

• Advanced lipid testing, including Lp(a), ApoB, NMR particle sizing, and insulin-resistance phenotyping

• Metabolic and nutritional profiling

• Microbiome analysis

• Heavy-metal testing

• Frailty assessment

• Cognitive and functional assessments

• Circadian rhythm and sleep assessment

• Wearable-device and longitudinal health-data integration

• Advanced imaging, including total-body MRI when clinically appropriate

• Preventive health and lifestyle assessment

The NP will not simply collect test results; they will helpconnect the information across multiple clinical domains to develop a comprehensive picture of each participant's health and risk profile.

Participant Care

The MPHI NP will play a central role in creating an exceptional participant experience.

The NP will help participants understand:

• What their testing results mean

• Which findings represent potential health risks

• What actions may reduce those risks

• How lifestyle factors may influence their health

• When additional testing or specialty evaluation is appropriate

• How their genetic and pharmacogenomic information may affect care

• How their personalized health plan can be incorporated into their everyday lives

The goal is to transform complex precision-health information intoclear, meaningful, and achievable health strategies.

Collaboration & Care Coordination

The NP will work collaboratively with:

• MPHI Medical Director

• Nurse practitioners and physician assistants

• Health coaches

• Patient navigators

• Clinical technicians

• Meritus primary care physicians

• Meritus specialty physicians

• Pharmacists and medication-management teams

• Meritus School of Osteopathic Medicine faculty

• Research and clinical research teams

• Clinical informatics and Epic teams

• Community and population-health partners

The NP will serve as an important clinical bridge between MPHI and the participant's existing healthcare team, ensuring that clinically meaningful findings are communicated and appropriately incorporated into ongoing care.

Research & Academic Collaboration

Because MPHI is designed as both a clinical program and a longitudinal population-health initiative, the NP will have opportunities to participate in research activities.

Responsibilities may include:

• Supporting collection of high-quality clinical data for approved studies

• Participating in research protocols and longitudinal follow-up

• Assisting with quality and outcomes data collection

• Collaborating with MSOM faculty and research personnel

• Contributing to abstracts, posters, presentations, and publications when appropriate

• Supporting grant-funded initiatives

• Participating in development and evaluation of new precision-health clinical models

Qualifications

Required:

• Graduate degree from an accredited Nurse Practitioner program

• Current Maryland APRN licensure or eligibility for Maryland licensure

• National board certification as a Nurse Practitioner

• Current or eligible Maryland prescriptive authority

• Current BLS certification

• Demonstrated clinical experience providing comprehensive patient care

• Strong clinical assessment and communication skills

• Ability to work effectively within a multidisciplinary team

Strongly Preferred:

• Board certification in Family Medicine, Adult-Gerontology Primary Care, Adult-Gerontology Acute Care, or another primary care-related specialty

• Experience in preventive medicine, population health, or primary care

• Experience with lifestyle medicine or health optimization

• Familiarity with genetics, genomics, or pharmacogenomics

• Experience interpreting advanced laboratory or diagnostic testing

• Experience with Epic or another enterprise electronic health record

• Experience with clinical informatics or clinical decision-support systems

• Research experience or interest in clinical research

• Experience working with underserved or rural populations

• Experience with chronic disease prevention and risk-factor modification
