The Social Work Care Coordination Manager is responsible for overseeing the coordination of social work care coordination for patients in either the inpatient or outpatient setting. The manager ensures the delivery of high-quality, patient-centered care while optimizing operational efficiency. The Manager plays a pivotal role in managing staff, streamlining workflows, and ensuring adherence to healthcare standards and regulations.Licensure, Certification, or Registration Requirements for Hire: Licensed Clinical Social Worker (LCSW) in VirginiaLicensure, Certification, or Registration Requirements for continued employment: Current LCSWExperience REQUIRED: Minimum of seven (7) years of work experience in a medical setting to include two (2) years in a lead or supervisory role Previous work experience identifying and utilizing community resourcesExperience PREFERRED Previous work experience in an academic medical centerEducation/training REQUIRED: Masters Degree in Social Work from a Council on Social Work Education (CSWE) accredited programEducation/training PREFERRED: N/AIndependent action(s) required: Must be able to conduct duties and problem solve independently.Supervisory responsibilities (if applicable): Supervise over 30 Social Workers. $91,395.20 - $141,648.00Actual salary offers will be based on several key factors to include relevant work experience, credentials, and qualifications.
