POSITION:   CARE COORDINATOR, LEVEL I

MANAGING SUPERVISOR: DIRECTOR OF CLINICAL SUPPORT SERVICES OPERATIONS

PRIMARY CLINICAL SUPERVISOR: COMMUNITY CLINICAL PROGRAM DIRECTOR

DESCRIPTION OF JOB RESPONSIBILITIES AND DUTIES

Provide comprehensive case management and rehabilitation services to adults with severe psychiatric disabilities in Contra Costa County. The care coordinator will coordinate and provide rehabilitation, linkage, and case management services to these individuals in their natural settings (community, homes, shelter, park, hospital, etc.) and at the office.  Our consumers experience psychiatric hospitalizations, housing instability, homelessness, severe and acute mental illness, and money management difficulties.  This program will take a do-what-it-takes attitude to support those we serve to find hope, stability, and recovery.

MINIMUM REQUIREMENTS

  1. High school diploma
  2. Demonstrate flexibility to handle diverse and changing organizational needs
  3. Must be willing to work full time, minimum 40 hours/week, five days a week, in the clinic and community. Must be willing to work evenings/weekends as needed.
  4. Demonstrate primary professional commitment and knowledge for representation of The Hume Center. 
  5. Proficient in using GSuite (including Gmail, Drive, Sheets, Slides, etc), various Microsoft programs (including Word, Excel, and PowerPoint), and Electronic Health Records (EHR).
  6. Proficiency in Electronic Health Records preferred
  7. Experience working effectively with culturally, socio economically, and ethnically diverse populations.
  8. Possess a valid California Driver’s license. Must be willing to use your own vehicle and valid car insurance for work related travel. This job requires travel between clinics and community sites in Contra Costa and/or Alameda Counties at times. Transporting patients in your car and sometimes in a staff van. Mileage reimbursement is paid for all work-related travel.
  9. Strong commitment to respecting consumers and their family members and having concrete ideas about how to improve the meaningful involvement and participation of consumers in the program’s services.
  10. Strong commitment to community-based services for persons with high needs and low resources.
  11. Some knowledge of psychiatric disability and the navigation of the system of care for those with mental illness in the local area
  12. Be willing to use your own cell phone to be accessible and willing to be available to receive calls for crisis and/or emergency situations with supervisors and relevant colleagues.
  13. A history of maintaining confidentiality and discretion in all professional relationships
  14. Familiar with and/or able to work within the Family Partner and Consumer Empowerment shift in Psychology.
  15. Ability to pass pre-employment background check, Physical, and TB clearance

PREFERRED REQUIREMENTS

  1. Experience in a community behavioral health setting
  2. AA in Social Work, Counseling, Psychology or related field with several years’ professional experience in behavioral health
  3. Credentialed, or eligible to become credentialed, as a Mental Health Rehabilitation Specialist (MHRS).
  4. Proficiency in communication in any one of the following languages in addition to English: Spanish, Farsi, (Pushto/Dari), Punjabi, Hindi, Tamil, Urdu, Gujarati, Cantonese, Mandarin, Vietnamese, or ASL or other language spoken in the Bay Area.

KNOWLEDGE AND SKILLS

  1. Consult with staff on clinical and program matters as the need arises.
  2. Some understanding of alcohol and other drug addiction and recovery.

  4. Some understanding of basic interpersonal skills (e.g., attending & listening, exploring thoughts and feelings, curious questioning)
  5. Some understanding of about educational, judicial, health care, housing, shelters, emergency, and other social service systems in community
  6. Some understanding of clinical documentation and billing standards

DIRECT SERVICES

  1. Provide services based on psychological and psychiatric assessments.
  2. Assist housing applications, support consumers getting into appropriate housing, and provide supportive housing services once consumers are housed to help them maintain housing
  3. Provide services in service program(s), included, but not limited to, evaluation, rehabilitation, case management, collateral, crisis intervention, and consultation.
  4. Complete case records, including progress notes, discharge/transfer summaries, etc.
  5. Gather information for evaluation
  6. Perform all evaluations, interventions, and outcome measurements on-time, including data entry of the results as needed.
  7. Help link consumers to SSI, SDI, Food, Housing, Transportation, Primary Care, Psychosocial Rehab, Educational and Employment Supports and Opportunities, and all other services that will help support the consumer’s progress in treatment.
  8. Fulfill additional responsibilities as needed.
  9. Staff the crisis phone on a rotating basis with other staff

OTHER

  1. Support and cover for other team members as necessary.
  2. Meet billable hours set by The Hume Center.
  3. Attend weekly staff meetings, trainings, group and individual supervision.
  4. Participate in quality assurance processes within the program and department and ensure all documentation meets all relevant internal and external standards.
  5. Community outreach.
  6. Meet with the Supervisor on a weekly basis for individual supervision 
  7. Demonstrate an understanding of The Hume Center’s Philosophy, Organizational Culture, and  Organizational Channels of Communication through practicing an enabling model of working  together 
  8. Attend other required meetings 
  9. Fulfill any additional duties specified by Supervisor (Director and/or Vice-President) and President/CEO
