Schedule/Hours: Salaried 40 Hours Per Week

Remote position located in the Western Kentucky region (Madisonville, Paducah, Hopkinsville Etc.)

 

This position is responsible for:

 

Provides subject matter expertise in evaluating, supporting and coordinating healthcare and care needs assisting Veterans in gaining access to health care through VA or community providers, support health care plans by identifying and resolving barriers to care and providing education on wellness related topics. Position requires regular travel serving as a liaison between Veterans and health care and community care representatives. SSVF Health Care Navigator will not provide direct health care services. Navigators are not health care providers and do not deliver direct patient care. Navigators will not provide mental health counseling or make treatment recommendations.

 

Key Responsibilities:

 

  • Provides comprehensive case management and care coordination across episodes of care; serves as a health coach by proactively supporting the Veteran to Conducts assessments of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others. The purpose of the assessment is to understand the Veteran’s situation, potential barriers to care, the causes, and the impact of such barriers on the Veteran’s ability to access and maintain health care services. The assessment should highlight the Veteran's strengths, limitations, risk factors, and internal/external supports and service needs to optimize the Veteran's ability to access and maintain health care services. The initial assessment will be completed as specified by the policy of the SSVF grantee. An assessment may be accomplished through virtual technology

 

  • Participates in expanding the knowledge related to health care navigators and the Veteran population. 

 

  • Assists in identifying the Veteran and family's health education needs and provides education services and materials that match the health literacy level of the Veteran. 

 

Physical Requirements and Working Conditions:

 

Ability to travel and transport clients to medical appointments.

The position requires travel over a multi-county region and is required to spend up to 2 hours at a time conducting a vehicle. 

Ability to read, to write, and communicate the English language.

Ability to keypunch information into a computer.

Ability to sort and file documents alphabetically and numerically.

Ability to operate and answer the telephone.

Ability to operate various general office equipment including a computer and scanner.

This Position is required to do basic business and office work. In order to successfully perform the essential functions of this position, the individual is regularly required to talk and listen, stand, walk, sit and use hands. 

Individual must be able to regularly lift up to 30 pounds and occasionally lift up to fifty pounds. 

The use of Office and Computer Equipment is necessary. Vision abilities will require close vision. 

Work is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like

Work may be performed indoors and occasionally require outdoors to attend meetings. There is low to moderate noise level for this position

WHAT WE ARE LOOKING FOR IN YOU

  • Bachelor’s degree in social work or health care administration, or related field with a LCSW, Master’s level social worker or equivalent education and experience preferred AND

 

  • At least three years’ experience in care management of clients, experience with the VA Medical System preferred OR

 

  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

 

 

Licenses and Certifications:

Valid State Driver’s License
