POSITION SUMMARY

Under the direction of the Senior Director, Health & Wellbeing, the Health & Well-Being Lifestyle Coach facilitates delivery of the YMCA’s Diabetes Prevention Program and creates a welcoming, supportive environment where participants can build sustainable healthy habits. The Lifestyle Coach delivers the approved curriculum with fidelity, facilitates engaging group experiences, supports participant goal setting and behavior change, maintains required program documentation, and builds relationships that encourage participation and retention. The incumbent demonstrates the YMCA core values of caring, honesty, respect and responsibility in all aspects of their work.

ESSENTIAL FUNCTIONS

  • Deliver the YMCA’s Diabetes Prevention Program curriculum with fidelity in effective, meaningful and engaging ways.
  • Facilitate group participation and discussion using open-ended questions, active listening and other effective facilitation techniques.
  • Create a safe, welcoming, judgment-free and non-competitive environment that supports participant learning, belonging and behavior change.
  • Build relationships with and among participants and encourage continued participation and program retention.
  • Support participants in setting realistic goals, identifying barriers, celebrating progress and building sustainable healthy habits.
  • Prepare for each session by reviewing curriculum content, participant information and trackers, developing a facilitation plan, and completing participant reminders as appropriate.
  • Collect required participant information, including on-site weight information, in accordance with program procedures.
  • Accurately record required weekly participant and program data in the designated database within 48 hours of each session.
  • Be available to participants before and after sessions to answer questions and provide appropriate follow-up and encouragement.
  • Follow up with participants outside scheduled sessions and conduct in-person make-up sessions as needed to support participation and program requirements.
  • Protect participant information and follow YMCA privacy, confidentiality and HIPAA-related policies and procedures applicable to the program.
  • Travel to community locations as needed to deliver the program.
  • Participate in required Lifestyle Coach team meetings, communications, training and professional development opportunities.
  • Follow YMCA policies and procedures, including personnel, safety, facility access, membership and emergency procedures.
  • Attend and remain current on all mandatory training and support the YMCA’s mission and annual support campaign.

WORK ENVIRONMENT & PHYSICAL DEMANDS

  • May be required to transport, lift and/or move program materials weighing up to 25 pounds.
  • Work is performed in YMCA facilities and community locations; noise levels and physical environments may vary by program site.

Qualifications

QUALIFICATIONS

  • Strong communication, active listening, relationship-building and group facilitation skills.
  • Ability to create community within a group and demonstrate empathy, encouragement and respect for participants pursuing health and wellness goals.
  • Knowledge of basic health, nutrition, physical activity and wellness concepts preferred.
  • Ability to work effectively and inclusively with people of varied backgrounds, experiences and abilities; experience working with older adults is preferred.
  • Familiarity with Listen First, motivational interviewing or other participant-centered coaching approaches is highly encouraged.
  • Commitment to the YMCA mission and values, with an emphasis on supporting participants as they pursue sustainable health changes.
  • Current CPR and First Aid certification preferred.
  • Ability to complete and remain current on required YMCA Diabetes Prevention Program Lifestyle Coach training and other mandatory training.
