Community Health Worker - LCHC Burns Flat

Education:

• High school diploma or equivalent required.

• Associate or bachelor’s degree in public health, social work, human services, or a related field is preferred.

• Ongoing training in community health practices and health education is advantageous.
  
   

Regulatory Requirements:

• Valid driver’s license and proof of insurance, if the role involves travel.

• Background check and health screenings may be required depending on employer policies and funding sources.

• CPR/First Aid certification may be required or preferred.
  
   

Responsibilities:

• Conduct outreach and education in homes, clinics, community centers, and other local settings.

• Assist individuals and families in navigating healthcare and social service systems.

• Provide culturally appropriate health education and information.

• Help clients access medical, behavioral health, housing, food, and other essential services.

• Advocate on behalf of individuals and communities to reduce barriers to care.

• Build trusting relationships with clients to support health behavior change.

• Coordinate with healthcare providers and social service agencies for follow-up and care management.

• Document client interactions and maintain accurate records in compliance with privacy standards.

• Attend and participate in community events, training, and staff meetings.
