šŸ§‘ā€āš•ļø What You’ll Do as a Prescribing Pharmacist

We are looking for a Clinical Prescribing Pharmacist to support patients within Doccla’s virtual care model.
You will play a key role in medication optimisation, supporting patient safety, that will help to prevent exacerbations and acute hospital admissions, allowing patients to have better management of their long term condition symptoms.

āœ… What Experience You’ll Bring to the Team

  • Conductstructured medication reviews

  • Process and respond tospecialist treatment letters

  • Supportcare homes with medication-related needs

  • Assist withpractice dispensary operations

  • Processhospital discharge letters

  • Act as theprimary point of contact for all medication-related queries within the practice

  • Work collaboratively with GPs and wider healthcare professionals to ensure safe, effective prescribing

  • Support patient understanding and adherence through clear, tailored communication

šŸ’” What We’re Looking For

  • Independent Prescriber (IP) status

  • Completion of thePrimary Care Pharmacy Education Pathway (PCPEP)

  • Previous experience working within aGP practice

  • Minimum of three years’ post-graduate experience

  • Strong clinical knowledge and decision-making skills

  • Confidence working in abusy primary care environment

  • Ability to adapt communication style for both patients and healthcare professionals

  • Commitment to ongoing learning and professional development

  • Availability to work aminimum of 15 hours per week

  • Registered with theGeneral Pharmaceutical Council (GPhC)

  • PersonalProfessional Indemnity Insurance

Desirable

  • Clinical Diploma (beneficial but not essential)

  • Experience working with remote or digital healthcare models
