Schedule: PRN Mon-Fri (8 AM - 5PM) - 1 day per week

Beaumont Home Health is seeking a compassionate and dependableLPN to provide skilled nursing care to patients in their homes.

What You’ll Do

• Provide skilled nursing care according to physician orders and RN care plans

• Monitor patients and report changes to the RN
• Administer medications and treatments as prescribed
• Educate patients and families on care and disease management
• Complete accurate, timely documentation
• Communicate with patients, families, and the care team
• Participate in on-call responsibilities, meetings, and training
• Travel to patients’ homes within the assigned service area
   

Requirements

• Current LPN license in the state of employment

• 1+ year of acute care LPN experience required
• 2 years preferred; home health experience a plus
• Strong communication, organization, and clinical judgment
• Compassionate and patient-focused
• Valid driver’s license and auto insurance
