Position Overview

The DayHabilitation Specialist is responsible for implementingtherapeutic objectives as outlined in members'Day Habilitation Service Plans (DHSP). This role providesdirect supervision, training, and support to members in bothprogram and community settings. The Habilitation Specialist serves as a resource for members, families, and team members, ensuring a safe, engaging, and person-centered environment.

 

Key Responsibilities

Member Support & Program Implementation

  • Implement members'therapeutic objectives based on theirDay Habilitation Service Plan (DHSP) and ensure accuratedata collection and documentation of progress.
  • Support members in developingself-care, communication, self-direction, and daily living skills based on individual needs and learning styles.
  • Assist inpersonal care tasks, including wheelchair transfers, toileting, showering, and vehicle transfers, while maintaining a dignified and supportive environment.
  • Facilitatecommunity inclusion activities aligned with members’interests and therapeutic goals.
  • UsePositive Behavioral Supports to encourage appropriate behaviors and promote skill development.

Safety, Crisis Intervention & Compliance

  • Ensure adherence toHuman Rights and Sexuality Policies, maintaining asafe, respectful, and supportive environment for all members.
  • Act as arole model, demonstrating professionalism indress, language, and behavior.
  • ImplementHealth & Safety policies, including:
    • Familiarity withevacuation routes and emergency protocols.
    • Awareness offire safety procedures and proper use of fire alarm pull stations.
    • Ensuringclean and accessible aisles and exits.
    • Supervising members duringmeals, breaks, and transportation to community sites.
    • Performinglight housekeeping and training members in maintaininga clean and organized environment.
    • Usingsafe lifting and transferring techniques at all times.
  • Participate incrisis intervention, including:
    • Consistently implementingbehavioral support plans.
    • Responding effectively tocrisis situations.
    • ExecutingBehavior Management and Emergency Restraint interventions appropriately.
    • Completingincident reports and related documentation in an accurate and timely manner.

Qualifications

 

  • High school diploma or equivalent (related experience inHuman Services or Behavior Management may be considered in place of formal education).
  • Valid Driver’s License.
  • Successful completion ofall state and agency-mandated training.
  • Experience orworking knowledge of Positive Behavioral Supports is preferred.
  • Strongoral and written communication skills.
  • Must pass aCriminal History Background Check (CORI) and screenings fromDPPC, DDS, and other regulatory agencies.
  • Ability to performall essential job duties, with reasonable accommodations available for individuals with disabilities.

 

Language Skills

  • Fluent inEnglish with the ability toread, interpret, and write reports related tosafety rules, operational procedures, and program guidelines.
  • Effective communication skills to interact withmembers, staff, families, and community partners.
  • Ability topresent information clearly in bothindividual and small group settings.

Physical Demands

  • Ability tostand, walk, stoop, kneel, and crouch for extended periods.
  • Frequentuse of hands and arms for handling tools, assisting members, and performing essential tasks.
  • Ability tolift and maneuver up to 100 pounds (with assistance when necessary).
  • Ability tolift 30 pounds from floor to waist and 10-15 pounds overhead.
  • Significantphysical endurance required to sustain the job’s demands.
  • Reasonable accommodations available for individuals with disabilities.

Reasoning Ability

  • Ability tounderstand and follow detailed written or verbal instructions.
