Job Description: Case ManagerĀ - School & Office Based
Requirements:
- High School Diploma RequiredĀ
- Bachelorās Degree, preferred
- 1-2 year experience preferred
Responsibilities:
General:Ā
- Maintain a working knowledge of services that are appropriate for the needs of the person served and support systems that are relevant to the life of the person served.
- Deliver culturally competent services to clients.
- Maintain professional objectivity and self-discipline in conducting job responsibilities ensuring each youthās needs remain the primary focus.
- Ensure that level of assistance and service delivery is provided based on the client's strengths, needs, abilities and preferences.
- Make recommendations and referrals as appropriate for services.
- Complete all required documentation ensuring medical necessity and compliance with certification standards and Catalyst Counseling, LLCĀ policies.
- Attend and utilize supervisionĀ
- Review problems and concerns with the supervisor to ensure effective and efficient completion of essential responsibilities.
- Maintain compliance and adheres to all Catalyst Counseling, LLCĀ policies and practices, meeting critical expectations as defined.
With Clients and FamiliesĀ
- Assist clients and families in achieving stated goals and objectives, and maximizing independence and productivity through support and training in use of personal and community resources.
- Plan and conduct individual, family or group treatment interventions as indicated in the Individualized Service Plan.Ā
- Refer/link families with appropriate services in the community to meet their ongoing needs.
- Provide crisis response as needed consistent with behavior modification/de-escalation training and according to program procedures.
With Essential Others
- Involve and work cooperatively with family members, significant others and community partners in the treatment planning and review process.
