Join Our Team as a Case Manger!   Are you passionate about supporting justice involved population and making a real difference in their lives? EAC Network, a leading not-for-profit social service agency dedicated to helping over 80,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full time Case Manager to join the Community Services Program.
  • Wage: $20/ hour
  • Schedule:9AM-5PM, Monday- Friday
  • Location: 175 Fulton Ave 4th Floor in Hempstead, NY, 11550
  What drives the core purpose of this job: The Case Manager is responsible for working directly with offenders involved with the Community Service Program. The Case Manager conducts screenings/intake interviews, refers clients to appropriate volunteer worksites, and monitors these individuals to complete.     What are the key responsibilities:
  1. Conduct client intake interviews and collect client fees.
  2. Match clients to appropriate worksite placements and conduct client follow-up.
  3. Identify and link clients with supportive services.
  4. Maintain accurate client records and have available for inspection by Supervision at any time.
  5. Log information in the program database per established timeframe.
  6. Monitor clients while they are performing their community service assignment and report any problems, issues or concerns regarding the clients progress.
  7. Submit client completion forms and other required for submission to the community justice referral source.
  8. Assist in maintaining information systems and tracking of referrals.
  9. Provide required client information to ensure prompt entries into database.
  What key knowledge, skills, and abilities are required to excel in this role:
  1. High School Diploma or educational equivalent required, college degree preferred.
  2. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
  3. Excellent communication, organizational and interpersonal skills.
  4. Must possess a valid NY State Drivers License and have access to an automobile.
  5. Bi-lingual capability (English/Spanish) a plus.
