JOB DESCRIPTION
TITLE: OPTIONSWAIT LIST CARE MANAGER – Long Term Care Options (LTCO)
DEFINITION:Reporting to a supervisor, this position is responsible for care management of individuals on the OPTIONS Wait List and individuals who are receiving home-delivered meals while waiting for case assignment for other services. This position conducts semi-annual home visits and annual reassessments to determine any changes to consumers’ health and social status, provides information about other community resources, and updates Oracle and SAMS data bases. Position may provide regular telephone contacts as needed.
QUALIFICATIONS:
Education:BA/BS/BSW including at least 12 college-level credit hours in sociology, social welfare, psychology, or gerontology.
Experience:Minimum of one year social work or care management experience in the fields of aging, behavioral health, or disabilities.

• Well-developed interpersonal and communication skills.
• Well organized.
• High level of flexibility.
• Possesses advocacy skills and a sense of professional ethics.
• Strong computer skills
Additional Requirements:
• Must have a valid driver’s license, good driving record, and
continuous access to a fully-insured car
• Drug testing required
• Pre-employment physical
• Second language ability preferred
GOALS:
To maintain regular care management contact with individuals on the OPTIONS Wait List to determine and assess any changes in situation or needs that would impact placement on the wait list. To identify other supports or community resources that could be available to individuals. To monitor satisfaction with home delivered meals, as appropriate.
DUTIES AND FUNCTIONS:
a. Conducts annual in-home assessment of individuals on the OPTIONS Wait List using the Needs Assessment Tool (NAT).
b. Conducts home visit at 6 months to determine any changes in status or needs.
c. Determines that consumers receiving home-delivered meals are satisfied and receiving them as ordered
d. Provides information about appropriate, available community resources and facilitates referrals as indicated.
e. Assists consumers with Waiver enrollment applications as needed
f. May conduct required telephone contacts as needed to supplement OPTIONS Wait List Support Specialists.
g. Enters all required data into SAMS and Oracle.
h. Collaborates as needed with other OPTIONS Wait List team staff
i. Attends orientation and training, supervision, and team and staff meetings.
j. Other duties as assigned.

· Completes accurate Needs Assessment Tools
· Meets standards for contact requirements.
· Able to prioritize and complete assignments independently.
· Records, forms, and electronic data entry are timely, accurate, and complete.
· Assures quality services to consumers.
CUSTOMERS:
· Consumers on the OPTIONS Wait List.
· PCA staff.
