Aides - Personal Support Worker -Field

Overview: The Personal Support Worker will provide one-on-one services to children and adults with developmental disabilitiesin the Home-Based Support Services program through DHS. Essential characteristics of self-motivation,flexibility and a concierge outlook to provide the highest quality services for our clients. 

Description/job Duties: Provide direct support to both children and adults with intellectual disabilities in the home-based program. Collaborate with client support team members and ensure that individuals exercise their rights and are treated with dignity and respect. Responsible for the implementation and documentation of individual goals,objectives, behavior tracking and interventions when applicable in the Implementation Strategy Plan and Behavior Support Plan ofeach individual served.Assist individuals with their activities of daily living (ADL), promoting independence including, but not limited to, household chores, recreational activities, physical fitness activities, grooming and dressing, moneymanagement and community integration.Know the whereabouts of individuals you are directly responsible for at all times.No client can be left unattended for any period of time during your shift. Attend training asrequired to continue skilldevelopment as necessary. Meet the needs of the individual served and their family/guardian(s). Report medical or behavioral concerns and accidents / incidentsimmediately to parent/guardian and your supervisor. Reportany and all allegations of individual abuse or neglect thatis reported to orwitnessedimmediately your supervisor and Office of Inspector General (OIG). Responsible for collecting data and filling out appropriatedocumentation each shift. Completion of state required DSP Certification within90 days of hire. Other dutiesassigned.Non Direct time will be spent inNaperville office. This can include but not limited to documentation, session prep,trainings, meeting with supervisor etc.  

Hours: Tuesday, Wednesday,ThursdayFriday 9:00am-2:00pm. MUST BE ABLE TO WORK FULL SCHEDULED LISTED. 

Location:Office is located in Naperville, IL. Client Located in all neighboring areas. 

Reports to: Clinical Manager 

Qualifications:  

Must be 21 years of age. 

Valid driver’s license in good standingrequired. 

Access to a registered vehicle with proof of insurance to provide transportation for individuals served. 

Ability to work with both children and adults.  

Current certified Direct Support Person (DSP) or completion of DSP certification upon hire.  

CPR and First Aid Certification or completion of certifications upon hire. 

Experience and understanding of individuals with developmental disabilities or mental health diagnoses. 

Experience with individuals whoexhibit maladaptive behaviors. 

Successful clearance of local and/or state background checks.
