About the Role
We are seeking afull-time Associate Care Manager to support patient engagement, care coordination, and remote patient monitoring efforts. This role is ideal for candidates witha background as a Medical Assistant (MA), Certified Nursing Assistant (CNA), or similar healthcare role with experience incare coordination, patient navigation, or remote patient monitoring (RPM).
TheAssociate Care Manager works closely with patients enrolled in Babyscripts' RPM programs, ensuring compliance with care plans, addressing questions, and escalating concerns to clinical staff when needed. While prior experience inmaternal health or OB care is a plus, it is not required. The ideal candidate is passionate about patient engagement, digital health, and improving healthcare outcomes.

Key Responsibilities
- Engage with patients through in-app messaging, phone calls, and telehealth check-ins to support adherence to their care plans.
- Monitor patient data (such as blood pressure readings) and conduct follow-ups with patients who have not submitted readings in several days.
- Educate patients on how to use digital health tools and properly measure vital signs.
- Escalate concerns related to abnormal patient readings or reported symptoms to RN Care Managers or clinical teams based on established protocols.
- Provide care coordination support, including appointment scheduling and answering routine patient questions.
- Assist in patient onboarding for Babyscripts' RPM program, ensuring patients understand expectations and have access to necessary resources.
- Document all interactions accurately in Babyscripts' care management platform.
Preferred Qualifications & Experience
- Experience incare coordination, patient engagement, or remote patient monitoring (RPM) is highly preferred.
- Background as aMedical Assistant (MA), Certified Nursing Assistant (CNA), or similar healthcare role.
- Strongcommunication and interpersonal skills, with a patient-first mindset.
- Proficient withdigital health tools, EHR systems, and telehealth platforms.
- Ability to work independently in aremote environment while maintaining productivity.
- Comfortable withstructured but flexible scheduling, ensuring full-time work commitment.
- Bilingual (Spanish preferred) is a plus.
