​Position Summary

We are seeking alicensed Physical Therapist (PT) to providehigh-quality, patient-centered therapy services on a PRN basis. This role focuses on helping residents improvemobility, strength, and independence while working as part of an interdisciplinary care team.

Key Responsibilities

• Evaluate residents and develop individualizedtreatment plans
• Provide skilled therapy services perphysician orders and care plans
• Document evaluations, treatments, and progress notes in compliance with regulations
• Communicate effectively with physicians, nursing staff, and care team members
• SupervisePTAs, aides, and students in accordance with state practice acts
• Participate in care planning and interdisciplinary meetings
• Educate residents, families, and caregivers on therapy goals and discharge plans
• Conductstaff training and in-service education
• Maintain compliance with allstate, federal, and payer guidelines

Qualifications

• Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
• ActiveSouth Carolina Physical Therapist license (in good standing)
• Experience inskilled nursing or rehabilitation setting preferred
• Familiarity withPointClickCare and Casamba preferred
• Strong clinical, documentation, and communication skills
