TheHospitalCaseManager(RN) plays a critical role in coordinating patient-centered care throughout the hospitalization and transition process. This position offers the opportunity to positively impact patient outcomes by facilitating safe discharges, reducing readmissions,optimizingresourceutilization,andsupportingcontinuityofcareacrossthehealthcarecontinuum.Workinginacommunityhospitalsetting,youwillbuildstronginterdisciplinaryrelationshipswhilehelpingpatientsandfamiliesnavigatecomplexhealthcareneeds.

TheHospitalCaseManager(RN)Will:

• Coordinateandfacilitatepatient-centeredcareplans,collaboratingwithinterdisciplinaryteamstopromotequalityoutcomesandappropriateutilizationofhealthcareresources.
• Reviewadmissionsandcontinuedstaysusingestablishedutilizationmanagementcriteriaandproviderecommendationsregardinglengthofstay,levelofcare,andqualityinitiatives.
• Communicatewithinsuranceprovidersandpayerorganizationstosupportauthorizationprocessesandoptimizereimbursementopportunities.
• Leaddischargeplanningefforts,ensuringpatientsandfamiliesunderstandpost-acutecareoptions,availableresources,andtransitionplans.
• Identifypatientsatriskforreadmissionandimplementinterventionsdesignedtoimproveoutcomesandreduceavoidablehospitalizations.
• Collaboratewithphysicians,nursingstaff,socialworkers,andcommunityagenciestosupportseamlesstransitionsacrosscaresettings.
• Participateindailyinterdisciplinaryprogressionrounds,focusingontimelycareprogressionandachievementofexpectedlengthsofstay.
• Maintainaccurateandtimelydocumentationofutilizationreviewactivities,dischargeplanninginterventions,andpost-acutecarearrangementswithinthemedicalrecord.

RequiredSkills

• Bachelor'sDegreeinNursing(BSN)required.
• CurrentRegisteredNurse(RN)licenseinMassachusetts.
• Minimumof3-5yearsofrecentacutemedical-surgicalnursingexperience.
• Strongclinicalassessment,carecoordination,andinterdisciplinarycollaborationskills.
• Knowledgeofutilizationmanagement,dischargeplanning,andregulatoryrequirements.

PreferredQualifications&Skills

• 1-3yearsofCaseManagementexperienceinanacutecarehospitalsetting.
• Experiencewithutilizationreview,payercommunications,anddischargeplanning.
• CaseManagementcertification(ACM,CCM,orequivalent)preferred.
• Experienceworkingininterdisciplinaryhealthcareteamsandnavigatingcomplexpatientcaretransitions.
