TheHospitalCaseManager(RN) plays a critical role in coordinating patient-centered care throughout the hospitalization and transition process. This position offers the opportunity to positively impact patient outcomes by facilitating safe discharges, reducing readmissions,optimizingresourceutilization,andsupportingcontinuityofcareacrossthehealthcarecontinuum.Workinginacommunityhospitalsetting,youwillbuildstronginterdisciplinaryrelationshipswhilehelpingpatientsandfamiliesnavigatecomplexhealthcareneeds.
TheHospitalCaseManager(RN)Will:
- Coordinateandfacilitatepatient-centeredcareplans,collaboratingwithinterdisciplinaryteamstopromotequalityoutcomesandappropriateutilizationofhealthcareresources.
- Reviewadmissionsandcontinuedstaysusingestablishedutilizationmanagementcriteriaandproviderecommendationsregardinglengthofstay,levelofcare,andqualityinitiatives.
- Communicatewithinsuranceprovidersandpayerorganizationstosupportauthorizationprocessesandoptimizereimbursementopportunities.
- Leaddischargeplanningefforts,ensuringpatientsandfamiliesunderstandpost-acutecareoptions,availableresources,andtransitionplans.
- Identifypatientsatriskforreadmissionandimplementinterventionsdesignedtoimproveoutcomesandreduceavoidablehospitalizations.
- Collaboratewithphysicians,nursingstaff,socialworkers,andcommunityagenciestosupportseamlesstransitionsacrosscaresettings.
- Participateindailyinterdisciplinaryprogressionrounds,focusingontimelycareprogressionandachievementofexpectedlengthsofstay.
- Maintainaccurateandtimelydocumentationofutilizationreviewactivities,dischargeplanninginterventions,andpost-acutecarearrangementswithinthemedicalrecord.

RequiredSkills
- Bachelor'sDegreeinNursing(BSN)required.
- CurrentRegisteredNurse(RN)licenseinMassachusetts.
- Minimumof3-5yearsofrecentacutemedical-surgicalnursingexperience.
- Strongclinicalassessment,carecoordination,andinterdisciplinarycollaborationskills.
- Knowledgeofutilizationmanagement,dischargeplanning,andregulatoryrequirements.
PreferredQualifications&Skills
- 1-3yearsofCaseManagementexperienceinanacutecarehospitalsetting.
- Experiencewithutilizationreview,payercommunications,anddischargeplanning.
- CaseManagementcertification(ACM,CCM,orequivalent)preferred.
- Experienceworkingininterdisciplinaryhealthcareteamsandnavigatingcomplexpatientcaretransitions.
