🎯 Your Key Responsibilities:

  • Conduct comprehensive evaluations to assess functional needs, develop individualized treatment plans, deliver evidence-based interventions, and monitor progress to maximize patient independence, safety, and quality of life.
  • Deliver evidence-based occupational therapy in accordance with physician orders and the CMS Conditions of Participation (CoPs).
  • Ensure timely, accurate documentation and maintain compliance with CMS, payer, and agency standards.
  • Collaborate with patients, caregivers, physicians, and the interdisciplinary team to maximize function and independence.
  • Educate patients and caregivers on ADLs, home safety, adaptive equipment, fall prevention, and individualized Home Exercise Programs (HEPs) to promote safety, independence, and functional recovery..
  • Monitor patient progress, update treatment plans as needed, and communicate changes to the care team.

πŸ’Ό What We're Looking For:

  • Degree from an accredited Occupational Therapy program.
  • Current Michigan Occupational Therapist license in good standing.
  • Home health experience preferred (minimum one year).
  • Strong clinical judgment, critical thinking, and problem-solving skills.
  • Excellent communication, interpersonal, and patient education skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Reliable transportation, a valid driver's license, and current auto insurance.
