We're hiring compassionate Home Health Speech-Language Pathologists (SLPs) in Oakland, Macomb, Wayne and Washtenaw counties to provide personalized, one-on-one care that helps patients improve communication, cognition, swallowing, and overall quality of life in the comfort of their homes. πŸ’™ You Might Be a Great Fit If You:

  • Conduct comprehensive speech-language evaluations to assess communication, cognitive, and swallowing needs; develop individualized treatment plans, provide evidence-based interventions, and monitor patient progress.
  • Deliver skilled speech therapy services in accordance with physician orders and theCMS Conditions of Participation (CoPs).
  • Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
  • Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize communication, cognition, swallowing, safety, and functional independence.
  • Educate patients and caregivers on communication strategies, swallowing precautions, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs) to support functional recovery.
  • Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team.

πŸ’Ό What We're Looking For:

  • Degree from an accreditedSpeech-Language Pathology program.
  • CurrentMichigan Speech-Language Pathologist (SLP) license in good standing.
  • Home health experience preferred (minimum one year).
  • Strong clinical judgment, critical thinking, and problem-solving skills.
  • Excellent communication, interpersonal, and patient education skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Reliable transportation, a valid driver's license, and current auto insurance.
