🎯 Your Key Responsibilities

  • Conduct comprehensive psychosocial assessments to identify patient and family needs, develop individualized care plans, provide interventions, and monitor progress toward patient goals.
  • Deliver skilled medical social work services in accordance with physician orders and the CMS Conditions of Participation (CoPs).
  • Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
  • Collaborate with patients, caregivers, physicians, and the interdisciplinary team to promote safe, effective care and successful discharge planning.
  • Provide counseling, emotional support, caregiver education, crisis intervention, and referrals to community resources, financial assistance, transportation, Durable Medical Equipment, long-term care, and other support services.
  • Advocate for patients, address psychosocial barriers to care, and communicate changes in patient status to the care team.

💼 What We're Looking For

  • Master’s degree in social work (MSW) from an accredited program.
  • Current Michigan Social Work license (LMSW or LLMSW) in good standing.
  • Home health or medical social work experience preferred.
  • Strong assessment, critical thinking, crisis intervention, and problem-solving skills.
  • Excellent communication, counseling, and interpersonal skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Reliable transportation, a valid driver's license, and current auto insurance.
