The IV Infusion SpecialistAssessmentNurseis responsible foridentifying current and future patient care needs; being able torecognize normal versus abnormal body physiology.The role requires critical thinking, attention to detail and being able to prioritizeappropriate interventions.An essential bridgebetweenthe assessment coordinator,inter-departments andexternalstakeholders that include, butisnot limited to the Department of Community Health, health careproviders,andmedical institutions.This role performsfrequentanalysis,requiresexcellent tracking skills,organizational skills,analytical andexceptional communication skills. Must always be committed to providing responsive aidin a courteous andprofessional manner.Â

Primary Duties and Responsibilities:Â
- Performdifferent aspects of the nursing process and function within the scoop of safe nursing practices. Â
- Work in supervisory role aswarranted by Ace Care and its needs at the time appointed.Â
- Perform case management services for medically frail and compromised patients using best Case Management practices.Â
- Provide full MDS assessment for new and renewed members.Â
- Responsible forassisting with review of member assessments, implementations, and management of nursing services within the ACE Care scope.Â
- Provide disease management initiation and updates as neededÂ
- Responsible forassisting with the initiation, development andmaintaining plan of care.Â
- Assist in Identifying family/client needs and for providing supportive carein accordancewith the plan of care.Â
- Perform education tomaintain standard of care within the nursing and case management department. Â
- Maintain regular communication with Administrator, other RN and LPN staff and lend assistance concerning clientâs progress and problems.Â
- Must be available via phone or in person to resolve medical emergent situations.Â
- Follow up with compliance for physicianâs orders and treatments being provided by direct care providers Â
- Communicate as needed with clientâs physician, family members and client for any change in plan of care or services.Â
- Assist with Q.I program review as needed.Â
- Assist with provider meetings as neededÂ
- Provide support andparticipate in IDT weekly meetingsÂ
- Represent the assessment department at the WAM (Weekly Ace meeting)Â
- Accept responsibility for coordinating physical care of the client, or providing direct care as needed.Â
- Responsible for knowing and understanding policies and procedures thatgovern the EDWP formally the SOURCE program.Â
- Must function independently in the absence of the administrator tomaintain operation short term.Â
- Accepts other assignments asappropriate.Â
Required Education and ExperienceÂ
- RN Certification (LPN or RN)Â
- At leasttwo (2) yearsin a related fieldÂ
Preferred Education and ExperienceÂ
- Associates DegreeÂ

- At leastthree(3)yearsin a related filedÂ
Additional Eligibility QualificationsÂ
- Proficiency in Microsoft365withexpertise in MicrosoftOutlook,Word, and Excel Â
