Position Summary
The Analyst, Outreach Coordinator utilizes several tools to help close care gaps throughout the year. The Analyst, Outreach Coordinator will perform medical record reviews abstraction and interact with regional teams and other stakeholders to facilitate actions that improve healthcare outcomes such as increased preventive care compliance. The Analyst/Outreach Coordinator will be the subject matter expert regarding HEDIS measures.
Additional responsibilities to include but not limited to the following:
- Subject Matter Expert in Healthcare Effectiveness Data and Information Set (HEDISĀ®) improvement work
- SupportsĀ EMR provider engagement
- Active participation in workgroups
- Delivers quality information to internal and external constituents via written and verbal communication
- Actively seeks opportunities for process improvement resulting in efficient and effective work streams
- Support complex provider partners with HEDISĀ® gaps in care, remote medical record collection discussions and education.
- Work closely with EMR provider partners to enhance collaboration
- between provider and Aetna
- Provides support for HEDISĀ® quality initiatives
- Supports effective medical record capture for HEDISĀ® medical record review and abstraction of supplemental data
Required Qualifications
- Ā 3 years' recent and related health care experience in any setting
- 1 year of Medicare experience and/or 1 year of HEDIS medical record abstraction experience
- Ā Ability to use standard corporate software packages and applications
Preferred Qualifications
- Experience successfully performing in an environment where daily/weekly productivity metrics are expected
- Ā Ability to work independently
- Ā Strong communication skills
- Ā Experience with EMR systems
- Ā Recent and related experience working with HEDISĀ®
- Ā Medicare Stars experience or health insurance experience
- Ā Health care related experience in clinical, managed care or quality management
- Ā Understanding of CPT/CPTII/ICD9/ICD10 coding and medical terminology
Education
- Bachelorās Degree or equivalent work experience in Healthcare

The typical pay range for this role is:
$21.10 - $36.78This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. Ā The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. Ā This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Ā
