Position Details
Position: Licensed Vocational Nurse (LVN)
Employment Type: Full-Time
Shift: PM Shift
Location: Moraga Post Acute
Pay Rate:$38.00 per hour
Responsibilities
Provide compassionate, high-quality nursing care to residents in accordance with their individualized care plans.
Administer medications and treatments as ordered by physicians and in accordance with facility policies.
Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals.
Perform nursing assessments and document resident care accurately and timely.
Collaborate with CNAs, RNs, physicians, therapists, and other members of the healthcare team.
Assist with developing and implementing resident care plans.
Provide wound care and other treatments as directed.
Monitor vital signs and other clinical indicators as needed.
Maintain accurate and complete nursing documentation.
Communicate effectively with residents, families, and healthcare providers.
Promote resident safety, dignity, privacy, and independence.
Follow all facility policies, procedures, and applicable healthcare regulations.
Respond appropriately to emergencies and changes in resident condition.
Provide guidance and support to nursing assistants and other care team members as appropriate.
Perform other nursing-related duties as assigned.
Qualifications
Current and validCalifornia LVN license.
Previous skilled nursing or long-term care experience preferred.
Strong clinical, organizational, and communication skills.
Knowledge of medication administration, treatments, and nursing procedures.
Ability to recognize and respond appropriately to changes in resident condition.
Compassionate and respectful approach to resident care.
Ability to work effectively as part of a multidisciplinary team.
Dependable, professional, and able to work independently.
Strong attention to detail and accurate documentation skills.
Commitment to providing safe, quality, and person-centered care.
