Overview

Job Summary:

The Patient Placement Coordinator usesacquired nursing knowledge and experience tofacilitate the transfer and acceptance of patients from all facilities within the markets served. The position collects clinical information relative to a transfer request, and thenfacilitates the transfer by matching the patient’s clinical needs to the capabilities of the organization and unit, as well asfacilitatesappropriate bed placementin accordance with the current Transfer Center processes. The Patient Placement Coordinatordemonstrates initiative and problem-solving skills while working with multiple departments within the hospital to foster relationships with referring facilities, physicians, and hospital staff. The position works closely with the leadership team at BooneHealthtoidentify, trend, and report barriers to patient throughput. This placement coordinator will use customer service skills and professional communication standards to positively interact with peers, customers, and supervisors.

Job Responsibilities:

Coordinates transfer of emergent, urgent, and elective patients to Boone Health Hospital from all potential referral sources to include but not limited to hospitals, clinics,nursinghomes, and physician offices via a single call toinitiate acceptance andexpedite decision-makingrelative to a potential transfer.
PerformsTransferCenter call functions ina timely, customer-centered professional manner with the aid of a computer database consisting of facility service profiles, approved clinical protocols,established patient care protocols, and healthcare information.
Serves as a Boone Health mediator,liaison, and strong communicator with physicians (and their offices),nurse practitioners,physician’s assistants, nurses, and ancillary staffregarding all patient transfers, potential bed placements, andassists with coordinating patient movement from one facility to another.
Ensurestimely transfer,appropriate bed assignment, andprovideaccurate patient information toassist with the development ofan initial plan of care whileproviding follow up to all participants of the transfer.
Demonstrates the ability to process patient acuityutilizing critical thinking skills, clinical decision making, andacquired knowledge toassist in assessing situations andfacilitate patient flow through the continuum of care.
Demonstrates the ability to triage and prioritize patient information and communicatean accurate clinical presentation of the patient for accepting physicians and nurses.
Demonstrates knowledge of regulatory requirements such as EMTALA, Boone Ethics andCompliance policies, and quality initiatives;monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives.
Participates in process improvement initiatives designed to improve overall functions within the Transfer Center by tracking barrier trends to patient intakeflow
Performs Other Responsibilities as Assigned.

Minimum Qualifications:

Degrees: High School Diploma or GED
Experience: <2 years
Supervisor Experience: None
Licenses & Certifications: LPN or EMT or RN or Paramedic License

Preferred Qualifications:

Degrees: Associate Degree
Experience: 2-5 years
